New patients

If you are new to our program and would like to schedule a consultation or get more information, please contact us. We can help coordinate with your child's pediatric or family medicine provider.

Please note: All pediatric and adolescent referrals must come from a pediatric or family medicine provider.

Returning patients

If you are a parent of a returning patient, you may use myDH to communicate securely with your child's providers, request or change appointments, review your child's medical record, and more.

Note: A proxy account must be established before you can view your child's medical records via myDH.

For referring physicians

Every pediatric, adolescent, and young adult needs a referral from their pediatric or family medicine provider. To avoid delays in processing the referral, please include most recent office visit notes and growth charts.

For the safety of our minor trans and gender-diverse youth, please be sure that a parent or guardian is aware of the referral before sending it to our team.

Do not send in a referral if the patient is uncomfortable speaking to their parents or guardians about their gender identity.

If the patient is 18 years or older, please be sure the patient's contact information is on the referral.

If you are unsure whether a patient is appropriate for our clinic, please don't hesitate to contact our team to discuss recommendations.

  • Internal referrals should be sent via eDH with attention to:
    Pediatric Endocrinology, Pediatric and Adolescent Trans Health Team
  • External Referrals with office notes and growth charts should be faxed to our office with attention to:
    Pediatric Endocrinology, Pediatric and Adolescent Trans Health Team
