The Craniofacial Coordinator is the primary point of contact for the Face of a Child Program. Contact our Coordinator for recommendations about evaluation and treatment, or if you wish to make a referral to the team. Our Coordinator facilitates clinic visits to ensure that children receive the treatment they need, when they need it.
New patients
When possible, it is preferable to get a referral to the program from a primary care or other medical provider, including details about medical history and treatment needs. Learn more about your visit to the Face of a Child Program.
Returning patients
If you are a returning patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record, and more.
